Physical Therapist - Home and Community Program

Rehab Without Walls Neuro Rehabilitation is seeking an enthusiastic Physical Therapist to join our Home and Community Program. If you are passionate about providing functional rehabilitation in real-life settings like homes, schools, workplaces, and communities, this role is for you. You will go beyond traditional therapy environments to help clients navigate daily experiences such as cooking, grocery shopping, dining out, outdoor activities, and performing at work. This position offers an autonomous setting within a collaborative team of dynamic therapists.

Responsibilities:

As a Physical Therapist, your responsibilities will include, but are not limited to:

  • Performing evaluations and developing comprehensive treatment plans.
  • Regularly re-assessing the effectiveness of treatment plans.
  • Attending staffing meetings and family conferences.
  • Conducting discharge evaluations, making recommendations for home and community safety, and providing referral and educational resources.
  • Communicating patient needs and progress to the treatment team, physician, person receiving services, and family members.
  • Collaborating with other disciplines to ensure coordinated care and enhance patient outcomes.

Qualifications:

To be successful in this role, you will need:

  • A minimum of a bachelor’s degree in Physical Therapy from an accredited program.
  • A current, unrestricted license as a Physical Therapist in the state of practice.
  • Current Basic Life Support or Cardiopulmonary Resuscitation certification as per state regulations.
  • A minimum of one year of work experience as a physical therapist is preferred.
  • Demonstrated knowledge of rehabilitation techniques for complex neurological injuries.
  • Effective and professional verbal and written communication skills.
  • Ability to lift up to 50 pounds.
  • Capacity to move, lift, or transfer patients, potentially up to 100 pounds, following safety procedures.
  • Proficiency in fine motor skills and visual acuity.
  • Ability to walk and stand for extended periods.
  • Frequent pushing/pulling, bending/stooping, reaching, kneeling, and positioning.
  • A health screen or examination may be required to verify physical capability with or without reasonable accommodations.
