Job Summary
The Registered Nurse (RN) in Long Term Care maintains the delivery of quality care by ensuring the nursing care plan is followed. The RN supervises Licensed Practical Nurses, Certified Nursing Assistants, and orientees while completing nursing assessments, documentation, patient education, and discharge planning in support of safe patient outcomes. Candidates are required to bring their license on the first day.
Job Details
- Schedule: Nights, 12 shift hours; minimum 48 hours per week.
- Required Qualifications: Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) certification (required prior to first shift where applicable); minimum of two (2) years of bedside clinical experience; Long Term Care setting competency aligned with the population served criteria.
- Preferred Qualifications: Progressive Care Unit (PCU) or Stepdown experience.
Responsibilities
- Conduct nursing assessments on all patients within 8 hours of admission to support individualized plans of care.
- Provide timely patient information and status updates for team conferences.
- Complete accurate, succinct, and timely documentation of assessments, interventions, and patient responses in the medical record in accordance with policy.
- Participate in the patient rehabilitation program and deliver direct patient care using the nursing process (assess, plan, implement, evaluate) with emphasis on patient/family education.
- Initiate discharge planning and patient teaching from the day of admission.
- Communicate changes in patient status to support continuity of care and patient safety, including relevant clinical team members.
- Provide direction, training, and supervision for ancillary nursing personnel using problem-solving and prioritization skills.
- Ensure personal and environmental safety in accordance with hospital and nursing policies and procedures.
- Contribute to unit functioning by providing calm guidance and reassurance.
- Maintain communication with management and senior nursing staff regarding service needs such as supplies, equipment, and clinical training.
- Assume charge responsibility as requested following the orientation period.
- Support professional growth through continuing education and self-study.
- Prioritize activities and plan effective use of time to meet patient needs.
