Original Job Summary

Overview

The Social Worker - Transitional Housing Program at Walden Family Services is tasked with supporting nonminor dependents and pregnant or parenting nonminor dependents (NMD) to meet their personal and program goals. This role involves working closely with clients, placing agencies, and adjunct treatment members to facilitate resources and services.

Essential Duties and Responsibilities

  • Utilize knowledge of transitional housing programs and resources for transitional-aged foster youth.
  • Follow Community Care licensing regulations for THP+FC.
  • Conduct pre-placement appraisals and explain program expectations.
  • Gather intake information and develop needs and services plans.
  • Provide weekly consultation discussing goals, life skills, and support services.
  • Assist with managing client cash, personal property, and budgeting needs.
  • Coordinate referrals for additional services such as medical, therapy, and job training.
  • Document weekly progress and complete required reports.
  • Attend individual supervision sessions and facilitate trainings and support groups.
  • Offer on-call crisis intervention and function as duty-worker when needed.

Physical Demands & Work Environment

The job requires regular talking, hearing, sitting, standing, walking, bending, and occasional lifting of up to 15 pounds. The candidate must be prepared for frequent travel, computer work, and exposure to moderate noise and outdoor conditions.

Qualifications

  • A Master’s degree in social work or a related field.
  • A valid California Driver’s License, a good driving record, and access to a working vehicle.
  • Fingerprint and Child Abuse Index clearances from the Department of Justice.
  • Compliance with Community Care Licensing requirements.
