Overview

The Directing Registered Nurse eHomecare Nights Weekends Part-time at VON Canada manages pediatric, palliative, and acute medically complex clients with COPD or CHF. This remote role directs Personal Support Workers (PSWs) or Health Care Technicians (HCT) using a collaborative care approach.

Responsibilities

• Provide remote assessment and management of in-home care services
• Assign clinical tasks to PSWs/HCTs based on digital assessments
• Maintain accurate client records in both digital and paper formats
• Collaborate with multi-disciplinary team and external providers
• Participate in regular digital huddles, emails, and phone communications
• Guide and mentor team members while upholding VON and eHomecare standards
• Engage in internal projects and quality improvement initiatives

Qualifications

• Registered and in good standing with the College of Nurses of Ontario
• Certificate in Palliative Care or equivalent palliative education
• Extensive pediatric care experience (for Pediatric DRNs)
• Recent experience with palliative, pediatric, or acute COPD/CHF cases
• Advanced assessment, clinical leadership and independent decision-making skills
• Experience in digital health records, EMR, and mobile technology
• High-speed internet access and ability to work remotely
• Valid driver’s license and access to a personal vehicle
• Successful security screening and criminal records check

Working Conditions

The role requires attention to detail, physical activity such as lifting, carrying, and fine motor tasks along with walking, sitting, standing and climbing stairs. Good body mechanics are essential.
