Overview
The Remote Patient Monitoring Registered Nurse Clinical Care Manager plays a critical role in VNS Health's Care Management Program by collecting and analyzing biometric data to monitor patient health status. The role involves telephonic clinical assessments, patient education, and proactive care coordination to ensure timely interventions and optimal health outcomes.

What You Will Do
- Biometric Data Collection & Monitoring: Collect and review patient biometric data to identify health status changes.
- Clinical Assessment & Education: Conduct telephonic assessments and provide patient education based on clinical findings.
- Alert Response & Care Coordination: Respond to alerts using evidence-based triage strategies for timely care.
- Program Development & Collaboration: Partner with care teams, vendors, and health plans to support program outcomes.
- Quality Improvement: Analyze data to identify trends and enhance program quality.
- Professional Development: Engage in ongoing education to stay current with telehealth technologies and best practices.
Qualifications
Licenses and Certifications: Active and unrestricted Registered Nurse license in New York State.
Education: Associate's Degree in Nursing required; BSN preferred and from an accredited program.
Work Experience: Minimum 3 years as a Registered Nurse with at least 2 years in care management. Telehealth and geriatric care experience are a plus.
Skills: Strong analytical and clinical assessment skills, excellent communication, proficiency in telehealth platforms, and bilingual abilities may be required.
Pay Range
USD $49.55 - USD $61.96 per hour.
