About the Continuity Clinician Nurse Practitioner Role

Key Responsibilities
- Provide highest quality patient-centered care.
- Manage a panel of patients through continuous remote care.
- Collaborate with health coaches and expert clinicians.
- Ensure proactive follow up and share after-hours coverage.
- Participate in developing technology and care protocols.
Must-Haves
- Unrestricted medical license with willingness to obtain others.
- Board certification in Internal Medicine, Family Medicine, or Endocrinology.
- Minimum 2 years experience in diabetes management.
- Excellent organizational skills and outstanding bedside manner.
- Telemedicine experience preferred.

Additional Information
This position is remote-first with office hubs in Denver and San Francisco. Virta uses a location-based compensation model. For this role, the estimated compensation is between $104,400 and $120,000 per year, with occurrence malpractice insurance provided. Please note, clinical hiring restrictions apply in certain states.
