Reporting & Position

This position reports directly to the West Los Angeles Campus VASH Team Supervisor and Project Director.

Responsibilities

  • Provide case management and care coordination for veterans in VASH.
  • Assist with urgent client issues and after-hours support as needed.
  • Assess veterans holistically including housing, employment, legal, and health needs.
  • Maintain confidentiality in compliance with HIPAA standards.
  • Supervise student interns and assist with training new staff.
  • Conduct in-home, community, and office visits, documenting client progress accurately.
  • Perform data entry into HMIS, HOMES systems and complete accurate reporting.
  • Hold valid driver’s license, complete driver training and drive company vehicles.

Qualifications

  • Master’s Degree in Social Work, Psychology, Counseling or related field required.
  • Experience in social services; working with homeless or veterans preferred.
  • Ability to work independently and collaboratively.
  • Strong oral and written communication skills.
  • Must pass background investigation and credentialing process through VA’s VetPro.
  • Computer proficient in Microsoft Office and Internet applications.
