Reporting & Position
This position reports directly to the West Los Angeles Campus VASH Team Supervisor and Project Director.
Responsibilities
- Provide case management and care coordination for veterans in VASH.
- Assist with urgent client issues and after-hours support as needed.
- Assess veterans holistically including housing, employment, legal, and health needs.
- Maintain confidentiality in compliance with HIPAA standards.
- Supervise student interns and assist with training new staff.
- Conduct in-home, community, and office visits, documenting client progress accurately.
- Perform data entry into HMIS, HOMES systems and complete accurate reporting.
- Hold valid driver’s license, complete driver training and drive company vehicles.
Qualifications
- Master’s Degree in Social Work, Psychology, Counseling or related field required.
- Experience in social services; working with homeless or veterans preferred.
- Ability to work independently and collaboratively.
- Strong oral and written communication skills.
- Must pass background investigation and credentialing process through VA’s VetPro.
- Computer proficient in Microsoft Office and Internet applications.
