The Registered Nurse - Home Based Primary Care at U.S. Department of Veterans Affairs provides evidence-based, patient-centered care to veterans in their homes. This role includes oversight of licensed vocational nurses and nursing assistants, direct veteran care, and coordinated case management.
Main Duties
Key responsibilities include:
- Providing direct, competent care to veterans at home.
- Conducting case management, assessment, and triage.
- Coordinating medication, treatment, and care planning.
- Performing home visits with a government vehicle.
- Overseeing nursing education, orientation, and quality improvement.
In addition, this role emphasizes holistic care including health promotion, palliative care, and geriatric services while reducing unnecessary urgent care visits.
Benefits & Work Schedule
This is a full-time role with a competitive salary and comprehensive total rewards package including 50 days of paid time off, federal pension, 401K, and various insurance options.
Work Schedule: Monday - Friday, 0730 - 1600. No weekends or holidays. Telework and virtual options are not available.
Qualifications & Requirements
Candidates must have a full and unrestricted RN license, a degree in nursing from an accredited institution, and proficiency in English. Case management experience is preferred. Pre-employment physical and background checks are required.
