Job Description
Summary
The Pharmacist Clinical Specialist functions at the highest level of clinical practice as a mid-level provider. Working independently under the defined scope of practice, this role involves initiating, modifying, and discontinuing medication therapies, managing patient care through evidence-based regimens, and serving as a consultant in intensive medication therapy management.
Qualifications
Candidates must be graduates of an ACPE accredited College or School of Pharmacy with a Pharm.D. or BS Pharmacy, and hold a current unrestricted pharmacy license. Experience at the GS-13 level requires at least one year of experience as a GS-12, including budgetary execution, clinical decision-making, and therapeutic management. Residency training and experience in anticoagulation management within VHA is preferred.
Duties
- Conduct patient-specific pharmaceutical care from adult to geriatric populations.
- Initiate evidence-based pharmacotherapeutic regimens and monitoring plans.
- Perform medication reconciliation, physical assessments, and laboratory test interpretations.
- Monitor patient outcomes and document care plans accurately.
- Collaborate in research, formulary management, and quality assurance programs.
- Coordinate distributive functions and mentor staff within clinical settings.
- Engage in administrative functions to enhance pharmacy services.
