Overview
The Clinical Pharmacist Specialist Whole Health at U.S. Department of Veterans Affairs offers comprehensive medication therapy management, direct patient care, and active involvement in interdisciplinary teams to optimize patient outcomes.
Major Duties
- Design and monitor individualized therapeutic drug plans
- Provide direct patient care and medication therapy management
- Evaluate medication regimens for safety, efficacy, interactions, and adherence
- Order, interpret, and act on lab tests to support therapeutic adjustments
- Educate patients and families on medication use and disease management
- Manage adverse drug reactions, medication errors, and compliance issues
- Conduct telehealth assessments using video technologies
- Maintain accurate documentation of clinical interventions
- Participate in formulary compliance, interdisciplinary teams, and Whole Health initiatives
- Precept students, residents, and new staff; support clinical research
Work Schedule & Conditions
Monday-Friday, 8:00am - 4:30pm with ad-hoc telework. This is not a virtual position.
Qualifications
Applicants must be U.S. Citizens with a Pharm.D. or BS Pharmacy, and a full, current, and unrestricted license to practice pharmacy in the United States. GS-13 grade requires additional relevant experience.
Additional Information
This role requires maintaining licensure, participation in continuing education, and meeting physical and compliance requirements per VA standards.
