Summary

The Clinical Pharmacist Outpatient is the primary pharmacy staff member responsible for the daily operation of the pharmacy medication management systems at an affiliated tertiary care, teaching, medical, surgical, research hospital, and outpatient pharmacy.

Duties

The Clinical Pharmacist practices independently according to VA regulations, state and federal laws, and facility policies. Duties include providing pharmaceutical care and pharmacotherapy services that optimize drug therapy and resource utilization for both inpatients and outpatients.

  • Participate in multidisciplinary team rounds and provide drug information.
  • Evaluate patient therapy and document care plans in medical records.
  • Provide consultative services and recommendations to clinical staff.
  • Monitor drug therapy and communicate recommendations to prescribers.
  • Engage in quality assurance and outcomes research projects.

Work Schedule & Environment

Various shifts are available including early, mid, and late shifts with rotating weekends and holidays. This is an on-site position with no telework or virtual options.

Requirements & Qualifications

Candidates must be U.S. Citizens with a degree from an ACPE-accredited College of Pharmacy (BS Pharmacy or Pharm.D.) and hold a full, current, and unrestricted pharmacy license. Experience in outpatient pharmacy, non-sterile compounding, and direct patient counseling is preferred. Applicants must pass pre-employment physicals, background checks, and urinalysis screens among other requirements.

Additional Information

This GS12 position with the U.S. Department of Veterans Affairs is a federal role with comprehensive benefits. A trial period of 1 or 2 years is required due to the evaluated public interest in performance and conduct.
