Overview

The University of Washington is seeking a Clinical Pharmacist for their Anticoagulation Clinic at UW Medicine Center – Northwest. This is a part-time role (20 hours per week) on day shift focused on managing anticoagulant therapy for ambulatory patients.

Department and Team

Join the Ambulatory Pharmacy Department where a dedicated team of specialists in antithrombotic therapy work alongside multidisciplinary providers. The department emphasizes high-quality, comprehensive pharmacy services in collaboration with doctors, nurses, and healthcare students.

Primary Responsibilities

  • Implement and adjust antithrombotic pharmacotherapy for optimal therapy outcomes.
  • Monitor drug therapy including indications, dosing, side effects, and allergies.
  • Evaluate pharmacotherapeutic regimens for effectiveness and adverse events.
  • Provide patient education on anticoagulant therapy, dosing, and drug interactions.
  • Offer dosing recommendations and education to medical teams and staff.

Required Qualifications

Candidates must have one year of pharmacy practice experience or successful completion of an ASHP-accredited residency/fellowship. A valid Washington State Pharmacist License is required, along with prior anticoagulation experience being highly desirable.

Work Schedule and Compensation

This is a part-time, regular position working 20 hours per week during the day shift. The hourly pay range is between $66.46 and $96.62.
