Job Summary

Under the Department of Pharmaceutical Services, the Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist provides pharmaceutical care to patients at all ambulatory care pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist performs a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective. These include, for example, obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications, and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy.

The Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other clinic staff. The Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist will be based primarily in the ambulatory care setting managing specific patient population. Depending on the assigned shift and site, the Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist may also oversee pharmacy technicians to facilitate insurance authorizations for medications, as well as prepare and dispense prescription medications and pharmaceutical products for patient care. The Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist may also be responsible for the oversight of clinical activities, delivery of services, and the Patient Management Program. Oversight of the Patient Management Program is usually limited to when the Ambulatory Clinic Pharmacist - Hematology/Oncology Specialist is working a shift in collaboration with the UCSF Specialty Pharmacy.

This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs, as applicable.

Department Description

The Department of Pharmaceutical Services at UCSF has long been nationally recognized as one of the founding fathers for clinical pharmacy services, which was established in 1965. Today, the department continues to lead the profession through the integration of advanced technology & robotics, comprehensive pharmaceutical care and clinical services.

The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include, but are not limited to: drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department of Pharmaceutical Services is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.D. curriculum, clinical research, and public and clinical service.

Required Qualifications

  • Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy.
  • Must possess a current license in good standing to practice pharmacy in the State of California.
  • Current certification for Basic Life Support (BLS) from the American Heart Association.

Preferred Qualifications

  • Completion of an ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years of equivalent experience in ambulatory care.
  • Completion of an ASHP-accredited PGY2 Ambulatory Pharmacy Residency in relevant area, or at least 3 years of equivalent experience in ambulatory care.
  • Advanced Board certification (e.g., BPS).
  • Experience with navigating billing options for pharmacist care.
  • Experience precepting and evaluating pharmacy learners (e.g., residents and students).

License/Certification

  • Must possess a current license in good standing to practice pharmacy in the State of California.
  • Current certification for Basic Life Support (BLS) from the American Heart Association.
