Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated Registered Nurse (RN) Home Health.
Responsibilities:
- Oversee nursing care team.
- Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
- Plan, organize, and direct home care services.
- Conduct assessments of patient care needs and develop care plans.
- Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
- Prepare timely, complete, and accurate clinical notes.
Qualifications:
- Valid RN license in the State of Texas or compact license.
- Bilingual (Fluent in Spanish) encouraged to apply.
- Home health experience (at least 1 year).
- Knowledge of home health regulations and general healthcare compliance.
- Experience with Home Care Home Base (HCHB) EMR (preferred but not required).
- Excellent communication and people skills are essential.
- Ability to work independently and as part of a team.
