Occupational Therapist (OT) - Home Health
Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. We are a group of passionate people dedicated to delivering the highest quality services to our patients, families, team members, and the community. Our team is growing and looking for a dedicated Occupational Therapist (OT) for Home Health.
Responsibilities:
- Facilitate rehabilitation of patients with emotional, physical, or mental disabilities.
- Develop and implement occupational therapy care plans.
- Coordinate care with the home healthcare team.
- Evaluate therapy needs and monitor and report patient progress and outcomes.
- Educate patients and family members.
- Prepare timely, complete, and accurate clinical notes.
- Communicate effectively with patients, caregivers, healthcare providers, office support, and other care team members.
Qualifications:
- Occupational therapist license in the state of Texas.
- Home health experience is highly preferred.
- Knowledge of home health regulations and general healthcare compliance.
- Able to handle physical workload.
- Good driving record.
- Experience with HCHB EMR is preferred but not required.
- Effective communication and people skills.
