What To Expect (Job Responsibilities)
- Conduct outreach to assigned patients with chronic conditions via text, phone, and other systems for care coordination and support
- Document all patient interactions, goals, and care plans accurately and in a timely manner in the electronic health record (EHR) system
- Monitor patients' progress toward health goals and escalate concerns to clinical leadership as needed
- Serve as a resource and advocate for patients, assisting with medication adherence and appointment scheduling
- Collaborate with providers and care team members to ensure continuity and quality of care
What Is Required (Qualifications)
- Must be eligible to work for any employer in the U.S.
- High school diploma, GED, or suitable equivalent required; Associate's or Bachelor's Degrees in Healthcare or Biology strongly preferred
- Previous healthcare experience preferred
- Previous experience as a Medical Assistant or CNA desired
- Excellent verbal and written communication skills in English for effective patient care documentation
