Job Description

Community Health Nurse Interventionist

The Center for Latino Adolescent and Family Health (CLAFH) at The Johns Hopkins University is seeking a Community Health Nurse - Interventionist to join their research administration team in New York City. CLAFH is dedicated to eliminating health inequities and promoting the well-being of Latino adolescents and their families. While focused on the Latino community, the center's research and resources are also applicable to initiatives addressing health inequities in other populations. CLAFH's core mission involves developing, evaluating, and disseminating family-based interventions and nurse-driven healthcare models. These efforts aim to mitigate the impact of social determinants of health, reduce health disparities, increase healthcare engagement among Latinos, and enhance life opportunities.

Specific Duties And Responsibilities

  • Triage and conduct substance use/mental health screenings and clinical assessments for Latino and Black youth and their families, providing warm hand-offs to appropriate services.
  • Ensure care retention through individualized case management, needs assessment, counseling (via text, phone, or in-person), and family support in navigating the behavioral healthcare system.
  • Provide comprehensive coordination for each family's service package.
  • Implement brief and extended interventions for substance use, tailored to developmental age and aligned with risk levels.
  • Utilize brief, client-centered, non-judgmental motivational interviewing techniques to increase participant awareness of substance use and promote behavioral change.
  • Collaborate with community health workers and families to develop personalized substance use prevention and treatment plans for the entire household.

Minimum Qualifications

  • Graduation from an accredited school of nursing.
  • Licensure as an LPN in the state of New York.
  • Completion of the JHU credentialing process, including CPR certification, prior to the start date.
  • Additional education or experience may substitute for required qualifications as per the JHU equivalency formula.

  • Residency in the Bronx.
  • Bilingual and bi-cultural proficiency.
  • Flexible schedule, including weekends.
  • Demonstrated cultural competence working with Latino and African American communities.
  • Strong commitment to working with communities facing health inequities.
  • Ability to thrive in a fast-paced, deadline-driven environment.
  • Motivation, strong organizational skills, and attention to detail.
  • Willingness to work under the supervision of project directors and complete tasks efficiently.
