What To Expect (Job Responsibilities)
- Schedule patients for the entirety of visit plans at the beginning of each episode to ensure continuity of care
- Utilize productivity spreadsheets to maintain appropriate staffing levels and report deficiencies to the Clinical Manager
- Process incoming referrals accurately and strive to achieve admission within 24 hours of referral receipt
- Document updates on patient care and scheduling adjustments in communication notes
- Manage patient visits across all disciplines according to the plan of care and monitor underscheduled visits to prevent missed care
