St. Peter's Health Partners is seeking a dedicated Triage Nurse (RN II) for their busy Cardiology Office in Albany, NY. This full-time position offers a hybrid remote work arrangement after initial training. The role involves managing patient phone calls, assessing cardiac symptoms, providing patient education, and handling prescription refills and prior authorizations. What You Will Do

As a Registered Professional Nurse, you will utilize the nursing process to diagnose and treat patient needs. You will work collaboratively with cardiologists and colleagues to ensure excellent patient outcomes. Key responsibilities include:

  • Triaging patient phone calls and assessing cardiac symptoms.
  • Providing patient education via phone or nurse visits.
  • Managing prescription refills and prior authorizations.
  • Bringing patients to exam rooms, taking vital signs, and documenting in the Electronic Medical Record (EMR).
  • Reviewing and updating medication lists for accuracy.
  • Completing referrals and tracking patient compliance.
  • Reviewing and submitting prescriptions electronically for provider approval.
  • Obtaining patient consent for procedures.
  • Performing pre-visit planning and reviewing quality metrics.
  • Retrieving telephonic clinical information from patients.
  • Monitoring and completing tasks assigned by providers in a timely manner.
  • Educating patients on medications, testing procedures, and home care.
  • Ensuring proper handling and documentation of patient specimens.
  • Following up with patients regarding test results.
  • Maintaining a clean, safe, and disinfected work environment.
  • Collaborating with nursing colleagues to manage medical supply and drug cabinets.
  • Utilizing the EMR for effective communication.
  • Performing assigned quality assurance duties.
  • Providing clinical visit summaries and educational materials as requested.
  • Participating in daily Patient Care huddles.
  • Working cooperatively with all colleagues to ensure quality patient care.
  • Performing other assigned duties.

What You Will Need

  • Associate's or Bachelor's degree in nursing preferred.
  • High School Diploma or equivalent required.
  • Current, unencumbered NYS RN license.
  • Basic Life Support (BLS) certification.
  • Minimum of 6 months of previous RN experience.
  • Ability to lift up to 20 lbs.
