About the role
Registered Nurse Certified Diabetes Educator
St. Joseph’s Health Centre is seeking a highly motivated and self-directed Registered Nurse (RN), Certified Diabetes Educator (CDE) to join its dynamic interprofessional team within the Diabetes Management Program.
Our program supports a diverse and complex patient population living with diabetes and chronic disease. The successful candidate will provide high-quality, patient-centered care in a fast-paced outpatient setting, working collaboratively with endocrinologists and diabetes educators.
The RN, CDE will deliver education and clinical support to individuals with:
- Type 1 diabetes (MDI and insulin pump therapy)
- Type 2 diabetes with complex medical needs
- Diabetes in pregnancy
This role involves both independent and collaborative practice across clinic visits, one-on-one appointments, and group education sessions.
The successful candidate will also contribute to the advancement of nursing practice through knowledge sharing, continuous learning, and reflective practice.
Key Responsibilities
- Provide diabetes education and clinical services in outpatient, group, and virtual care settings
- Conduct comprehensive assessments and deliver patient and family education, incorporating social determinants of health
- Apply adult education principles and a variety of teaching methods to support patient self-management, including insulin titration
- Manage patients using:
- Insulin pump therapy
- Continuous Glucose Monitoring (CGM) systems
- Interpret clinical data and adjust care plans accordingly
- Communicate effectively with patients, families, and the interprofessional healthcare team
- Ensure continuity of care through:
- Accurate and timely documentation
- Coordination with primary care providers and other service providers
- Appropriate referrals
- Advocate for patients in addressing systemic barriers to care
- Support the development, implementation, and evaluation of individualized care plans
- Cross coverage on in-patient unit prn.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO)
- Certified Diabetes Educator (CDE) designation (required)
- Current Basic Cardiac Life Support (BCLS) certification
- Minimum 3–5 years recent experience in a Diabetes Management or Education Program
Clinical Experience
- Experience with Continuous Glucose Monitoring (CGM), including:
- Sensor insertion and patient training
- Data analysis and interpretation
- Experience with insulin pump therapy, including:
- Knowledge of current devices used in Canada
- Interpretation of pump-generated reports
Skills And Competencies
- Strong clinical judgment, critical thinking, and decision-making skills
- Excellent organizational and time-management abilities
- Ability to work independently and collaboratively in a fast-paced environment
- Strong interpersonal and communication skills
- Proficiency with Electronic Medical Records (EMR) (e.g., EPIC) is an asset
- Demonstrated reliability and strong attendance record
