The Role of a Registered Practical Nurse – Community Support Services
We are seeking a dedicated and compassionate Registered Practical Nurse – Community Support Services to join our team in Guelph. In this temporary part-time role, you will play a crucial part in providing high-quality nursing care to individuals within their homes and community settings. Your work will directly impact the lives of our clients, helping them maintain their independence, manage chronic conditions, and achieve optimal health outcomes through personalized and client-centered care.
Key Responsibilities
- Perform comprehensive health assessments, vital sign monitoring, and ongoing evaluation of client health status in various community settings.
- Administer medications, perform wound care, and execute other delegated medical procedures according to established care plans and professional standards.
- Develop, implement, and evaluate individualized care plans in collaboration with clients, families, and the interdisciplinary healthcare team.
- Provide health education and counseling to clients and their families on disease management, health promotion, and prevention strategies.
- Document all observations, interventions, and client responses accurately and promptly in compliance with organizational policies and regulatory requirements.
- Communicate effectively with physicians, case managers, and other healthcare professionals to ensure coordinated and seamless client care.
- Participate in team meetings, professional development opportunities, and quality improvement initiatives.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN).
- Graduation from an accredited Practical Nursing program.
- Minimum of one year of recent experience in community nursing, home care, or a related healthcare setting preferred.
- Valid driver's license and access to a reliable vehicle, as travel within the Guelph community is required.
- Current Basic Life Support (BLS) or CPR certification.
- Demonstrated ability to work independently and as part of a collaborative team.
- Excellent communication, interpersonal, and organizational skills.
- Proficiency in electronic health record systems and general computer applications.
