Pharmacist Staff at St. Elizabeth Healthcare

Engage with us for your next career opportunity at St. Elizabeth Healthcare. This role is a Regular, Part-time position with Scheduled Hours of 20 per week.

As a Pharmacist Staff, you will provide Pharmaceutical Care to patients with an emphasis on clinical monitoring and therapeutic interventions to improve patient outcomes. You will also be responsible for providing information and educational programs to customers and assisting with the maintenance of Performance Improvement processes to constantly enhance our Medication Use Process. A core responsibility is assuring dispensing accuracy and directing pharmacy workflow to ensure accurate and timely dispensing and distribution.

Job Duties and Responsibilities

  • Medication Order Verification: Review and interpret medication orders from licensed prescribers, ensuring completeness and appropriateness for the patient’s condition. Assess for drug interactions, allergy alerts, duplication of therapy, and potential therapeutic interactions. Obtain clarification from prescribers and provide appropriate therapeutic alternatives when necessary. Perform a second verification for pediatric and chemotherapy orders.
  • Therapeutic Clinical Monitoring: Actively participate in the ongoing monitoring of medication therapy in compliance with approved Collaborative Care Agreements and medication management protocols. Direct medication therapy within defined bounds, contact prescribers for therapeutic alternatives, and document assessments and recommendations in the patient’s chart using iVents and progress notes.
  • Compounds and Dispenses Pharmaceuticals: Ensure timely dispensing of appropriate quantities of correct medication. Supervise compounding according to USP standards, verifying ingredient compatibility, content, and appropriate labeling, including expiration dating. Check all medications prior to final dispensing and utilize barcode technology for accuracy. Supervise repackaging, including verification and documentation.
  • Supervises and Directs Interns and Technicians: Provide active direct supervision and mentoring of Interns and Technicians, monitoring task completion, redirecting staff to meet patient needs, and encouraging the use of the AIDET model. Report behavior not responsive to direct intervention to the leadership team.
  • Medication Use Safety Initiatives: Detect and report dispensing variances in a timely manner. Identify opportunities and participate in departmental efforts to increase the safe use of medications, such as clear labeling, new SALA medications, and Epic improvements. Identify and report suspected adverse drug reactions.
  • Controlled Substance Accountability: Participate in and supervise the receipt, dispensing, and distribution of controlled substances using systems like Pyxis C-II Safe, Pyxis Medstation 4000, Pyxis Anesthesia Stations, and internal perpetual inventories. Ensure accurate inventories, reconcile anesthesia waste, and resolve discrepancies.
  • Contributes to the Positive Overall Financial Status of the Department: Encourage prescriber compliance with formulary restrictions and therapeutic interchanges. Ensure formulary adherence, process non-formulary requests appropriately, and suggest therapeutic alternatives. Actively participate in IV to PO conversions and make efficient use of time to minimize overtime.
  • Drug Information / Staff Development: Provide accurate, adequate, and timely drug information to medical staff, nursing staff, and patients. Serve as a preceptor or advisor to Pharmacy Residents and Interns, and develop staff education programs.
  • Maintains Professional Competency: Maintain current US pharmacist licensure, complete all departmental competency/skills assessment requirements, and stay current with pharmacy practice. Seek to develop and maintain skills in specialized practice areas.
  • Perform other duties as assigned.

Qualifications

Education, Credentials, Licenses:

  • Graduate from an ACPE accredited pharmacy college
  • Current US pharmacist license in the state where work is being performed

Specialized Knowledge:

  • Problem solving and critical thinking skills
  • Good communication skills
  • Familiar with Current Practice Standards (ASHP)
