Job Summary

Develops clinical pharmacy practice skills consistent with ASHP accreditation standards. Collaborates with preceptors to optimize patient care by promoting rational, safe, and cost-effective drug therapy through developing and providing clinical services. This role is crucial for advancing patient care through expert pharmaceutical practice.

Job Responsibilities and Requirements

Primary Responsibilities:

• Provides pharmaceutical care in various clinical practice areas, aligned with preceptor guidance.
• Reviews and evaluates patient drug/medical history, offers drug therapy recommendations, and provides drug information.
• Evaluates medical literature, rounds with healthcare teams, and conducts drug utilization reviews.
• Documents medication-related plans and interventions in the electronic medical record.
• Ensures the safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring, and dose/therapy adjustment.
• Oversees the medication dispensing process, ensuring accurate and timely delivery of optimal pharmacotherapy.
• Supervises pharmacy technicians and/or interns involved in the drug delivery system.
• Contributes to a culture of learning and sharing to support professional development within the residency program.
• Implements staff development on pharmacy processes and participates in training for other healthcare professionals.
• Promotes interdisciplinary teamwork, developing and implementing clinical pathways and protocols for medication safety and utilization based on best practices and evidence.
• Works in a constant state of alertness and in a safe manner.
• Performs other duties as assigned.

EDUCATION

• Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an ACPE accredited school of pharmacy.

• No experience required.

Physical Requirements

• Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
• Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
• Frequent use of vision and depth perception for distances near and far, and to identify and distinguish colors.
• Frequent use of hearing and speech for oral communication.
• Ability to hear alarms and malfunctioning machinery.
• Frequent keyboard use/data entry.
• Occasional bending, stooping, kneeling, squatting, twisting and gripping.
• Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
• Rare climbing.

Required Professional License And/Or Certifications

• Resuscitation Certification Requirement: Basic Life Support Healthcare Provider (BLS HCP) required within 90 days of employment. Other resuscitation certifications may be required dependent on department.
