Join our team at SSM Health Verona Clinic as an Ambulatory Care Pharmacist Practitioner! We ensure positive patient outcomes by building meaningful patient relationships. You’ll lead Chronic Disease Management in Primary Care using broad CPAs. Schedule: Full Time, 40 hours, Day Shift. Hours: Monday through Friday typically 8:00 a.m. to 4:30 p.m. but may flex during clinic hours any time between 7:30 a.m. and 5:30 p.m. A one-time sign-on bonus may be available to qualified individuals with a 2-year service commitment.

Job Summary

The Pharmacist Clinical Specialist, Ambulatory assumes an expanded pharmacist role by providing direct patient care and may practice with approved privileges granted through collaborative practice agreements with Medical Staff. This role will be actively engaged in all complexities of patient medication management, monitoring, and education, as needed. The specialist also participates in the training and education of students, residents, and physicians as applicable.

Primary Responsibilities

  • Gathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapy.
  • Improves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.
  • Assesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as needed.
  • May design a drug therapy plan for patient-specific problems and independently implements the plan.
  • Provides enhanced patient care through detailed counseling on medications and disease states, adherence assessment, medication cost analysis, review of immunization status, and communication with patient’s providers and pharmacy.
  • Provides expert consultation with other health care professionals in the development of drug-related treatment plans, assessment of drug therapy, outcomes and identification and resolution of drug-related problems.
  • Identifies patient outcomes, correlates those outcomes to clinical processes, identifies processes that yield best outcomes, and assists in the implementation of those best practices.
  • Participates in quality management programs to facilitate continuous process improvement initiatives and quality improvement projects.
  • Precepts pharmacy students and residents, as applicable.
  • Works in a constant state of alertness and safe manner.
  • Performs other duties as assigned.

Education

Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy is required.

Experience

Two years of experience or completion of PGY1 residency is required.

Physical Requirements

The role involves frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs, frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors is necessary. Frequent use of hearing and speech to share information through oral communication and ability to hear alarms, malfunctioning machinery, etc. are also required. Frequent keyboard use/data entry is involved. Occasional bending, stooping, kneeling, squatting, twisting and gripping, as well as occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs, and rare climbing are part of the physical requirements.

Required Professional License And/Or Certifications

Basic Life Support Healthcare Provider (BLS HCP) is required within 90 days of employment. Other resuscitation certifications may be required dependent on department.

For Wisconsin location, a Pharmacist - Wisconsin Department of Safety and Professional Services license is required, along with one of the following Board Certifications from the Board of Pharmacy Specialties (BPS) or Specialty Pharmacy Certification Board (SPCB): Board Certified Ambulatory Care Pharmacist (BCACP), Board Certified Cardiology Pharmacist (BCCP), Board Certified Critical Care Pharmacist (BCCCP), Board Certified Geriatric Pharmacist (BCGP), Board Certified Infectious Diseases Pharmacist (BCIDP), Board Certified Nuclear Pharmacist (BCNP), Board Certified Nutrition Support Pharmacist (BCNSP), Board Certified Oncology Pharmacist (BCOP), Board Certified Pediatric Pharmacy Specialist (BCPPS), Board Certified Pharmacotherapy Specialist (BCPS), Board Certified Psychiatric Pharmacist (BCPP), Board Certified Sterile Compounding Pharmacist (BCSCP), Board Certified Transplant Pharmacist (BCTXP), or Certified Specialty Pharmacist (CSP).

Similar state-specific pharmacist licenses and board certifications are required for Illinois, Missouri, and Oklahoma where SSM Health operates.
