Physician Assistant or Nurse Practitioner House Calls - Emergency Medicine
Sollis Health is seeking an Advanced Care Provider- Physician Assistant or Nurse Practitioner to join our At Home, concierge house calls team. In this pivotal role, you will conduct concierge medical at-home visits for Sollis Health patients, providing care for urgent and emergent cases typically seen in an emergency room or clinic. You will reduce the need for patients to visit the emergency department by delivering care directly in their homes. This position is primarily focused in the New York area and is a Per Diem role, requiring 2-4 shifts per month.
Responsibilities
- Formulate differential diagnoses and treatment plans for urgent and emergent care within the scope of emergency medicine, to be administered in the patient’s home.
- Coordinate with House Call Coordinators for house call requests and respond promptly while on shift.
- Perform medical evaluations, utilizing in-home imaging studies as part of patient assessments.
- Ensure the house call provider bag is adequately stocked to prevent delays in patient care.
- Conduct Covid-related house calls, including obtaining clinical samples for testing, performing point-of-care testing, collecting blood for antibody tests, and providing symptom management. All Covid-related house calls require full PPE.
- Perform medical procedures such as obtaining IV access (including administration of fluids and medications), splinting, suturing, phlebotomy, performing POC testing, and performing EKGs.
- Document clinical evaluations and treatments in the electronic health record, eClinicalWorks.
- Provide patient-centered, empathetic communication, making patients comfortable and patiently answering their questions.
- Interface with the patient's PCP and specialists to discuss assessments, plans of care, and coordinate follow-up.
- Collaborate with the Medical Director and clinical team to ensure adherence to policies and deliver quality care in an innovative setting.
- Coordinate the hand-off of pending labs or imaging to in-clinic providers at the end of your shift.
- Coordinate additional care with the After Care, house calls teams, or Medical Partners.
- Occasionally cover shifts in Sollis' Flagship Centers and Clinics if an urgent need arises.
- Perform related duties as requested.
Experience
- Bachelor’s Degree, required.
- Graduate of an accredited PA or NP program, required.
- Physician Assistant or Nurse Practitioner state license and ability to practice in New York State.
- Preferred 2-3 years of Physician Assistant or Nurse Practitioner experience in emergency departments and/or urgent care centers.
- DEA license required.
- BLS, ACLS, PALS certification required upon hire.
- House call, EMS/pre-hospital, or concierge experience is a plus.
- Excellent clinical care and patient satisfaction in emergency department and urgent care settings.
Skills
- Strong clinical skills.
- Tech savvy with the ability to learn and use systems such as Slack, Athena Health, Salesforce, and Humanity.
- Superb technical skills, including phlebotomy, peripheral IV placement, and medication administration.
- Ability to be adaptable and work in a fluid, multitasking environment.
- Strong process and organizational skills.
- Ability to problem-solve and think outside the box.
- Comfortable interfacing with five-star clientele.
Compensation
Hourly rate range: $85 - $99 per hour. This range applies to New York candidates only. Actual pay may vary based on experience, location, education, etc.
