Job Overview
Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You will engage with people in the comfort of their own homes, helping to bridge gaps in care and build trusted relationships to make people healthier. This role brings flexible, comprehensive, and personalized health evaluations directly to people’s front doors.
Job Highlights
- Conduct approximately 45-minute in-home visits to connect one-on-one and answer health questions.
- Perform medication and medical history reviews, physical evaluations, and administer ordered diagnostic tests (e.g., diabetic eye exams, spirometry).
- Utilize an iPad with an intuitive clinical workflow; no prescribing, ordering labs, or altering current treatment regimens required.

Work Environment
As an in-home clinician, your primary workplace is the private residence of health plan members. You must be able to comfortably and safely perform your duties in diverse home environments, which may include the presence of household pets, secondhand smoke, and other potential airborne allergens or irritants.
