The Opportunity
Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in the comfort of their own homes to help bridge gaps in care and build trusted relationships to make people healthier.
Job Highlights
- The visit typically lasts about 45 minutes, allowing for in-depth, one-on-one connection and time to answer health questions.
- The evaluation includes a medication and medical history review, and a physical assessment.
- Additional diagnostic tests may be performed if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
- You will utilize an iPad with an intuitive clinical workflow for each evaluation.
- This role does not require prescribing medicine, ordering lab tests, or altering current treatment regimens.

- Be flexible to travel locally and within licensed states as needed.
- Conduct virtual visits when necessary.
- Provide all indicated Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening).

These environments are diverse and may present a variety of conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Pay Range
The Typical Pay Range For This Role Is $87,035.00 - $187,460.00. This pay range represents the base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors.
