Job Description

Case Manager - Adult Protective Services

Selfhelp Community Services is seeking a dedicated Case Manager to join their Adult Protective Services team. This role is crucial in assessing risk and capacity of referred individuals and addressing their needs through comprehensive care planning and advocacy.

Principal Responsibilities

  • Conduct assessments to determine client risk and capacity.
  • Establish appropriate, comprehensive care plans for clients.
  • Provide information and make referrals for services.
  • Follow up on all care plan items.
  • Ensure compliance with all New York State Office of Children and Family Services and New York City Human Resources Agency contractual obligations and agency policies.
  • Advocate on behalf of clients to ensure they receive entitled services.
  • Consult with and keep supervisor informed of client-related needs and problems.
  • Provide crisis intervention to stabilize client situations.
  • Deliver essential items such as cash, checks, food, and mail to clients as needed.
  • Maintain regular contact with clients, including monthly home visits to assess living conditions.
  • Participate in internal and external meetings to coordinate client needs.
  • Keep all involved individuals and agencies updated on client status.
  • Assess, initiate, and follow through on discharge planning.
  • Request, follow up on, and facilitate heavy-duty cleanings.
  • Attend and participate in court hearings.
  • Prepare and maintain required documentation, reports, applications, and statistics accurately and on time.
  • Maintain a professional demeanor with clients, peers, supervisors, and collaterals.
  • Document all interactions in case notes.
  • Ensure client files are updated, accurate, and complete.
  • Participate in supervision and training as assigned.
  • Provide on-call services during out-of-office hours on a rotating basis.

Job Competencies & Minimum Qualifications

  • Bachelor's Degree required.
  • One year of related experience and/or training preferred.
  • Working knowledge of Microsoft Office Suite and other technology.
  • Excellent communication and listening skills.
  • Excellent customer service skills with a focus on treating clients with respect and dignity.
  • Ability to multi-task and work independently with great attention to detail.

Working Conditions/Physical Demands

This role operates in a business office environment, requiring phone and computer use, and travel via public transportation. There is a potential for remote work 2-3 days per week.
