Registered Nurse-Physician Practice
Saratoga Hospital is seeking a dedicated Registered Nurse-Physician Practice to join their team. This role involves organizing and administering individualized, professional patient care within a primary care practice, adhering to hospital objectives, policies, and the state Nurse Practice Act. The ideal candidate will provide clinical leadership, foster a caring environment, and ensure patients and their families are at the center of the care process.
What You’ll Do
- Patient Assessment: Assess patient's physical/psycho-social status, review medications, confirm medication reconciliation, and obtain vital signs, height, and weight. Document findings and analyze data for care planning.
- Patient Care: Collaborate with providers to formulate and implement care plans. Refer patients to appropriate services, ensure adherence to Joint Commission standards, and triage patient calls. Advocate for patients and families within a relationship-based care model.
- Communication: Utilize culturally sensitive and population/age-specific communication skills. Accurately and concisely communicate pertinent information to providers. Manage conflict effectively within the team.
- Patient Education: Provide tailored information and educational materials to patients and families, assessing for learning barriers. Enhance patient and family education by consulting with the care team.
- Clinical Leadership: Delegate tasks appropriately to clinical staff, participate in problem-solving, and provide feedback to enhance team knowledge. Utilize critical thinking and evidence-based practice for clinical decision-making. Ensure staff practice within their scope.
- Documentation: Maintain accurate and compliant documentation in accordance with institutional standards.
What You Bring
- Graduate of an accredited school of nursing; BSN preferred.
- Licensed to practice in New York State.
- Ability to organize priorities, utilize critical thinking, multitask, and maintain professionalism.
- Excellent customer service, integrity, friendliness, and compassion.
- Effective interpersonal, communication, and telephone skills.
- Ability to establish rapport with patients, co-workers, and medical staff.
- Proficient in computer use for email and electronic medical records.
- Flexibility to take initiative and embrace new opportunities.
