Job Description

Clinical Resource Nurse at Rochester Regional Health

As a Clinical Resource Nurse at Rochester Regional Health, you will be an experienced registered nurse responsible for guiding, coaching, promoting, and executing quality care delivery across inpatient and outpatient units. This role is crucial for improving quality care, enhancing patient experience, and supporting staff development within a patient-centered care model.

You will play a vital role in patient care, team education, and upholding the highest standards of clinical excellence.

Key Responsibilities

  • Perform initial and ongoing patient assessments, utilizing appropriate physical, psychological, educational, and risk-screen data tailored to the patient's age.
  • Demonstrate expert clinical judgment in prioritizing and responding to patient needs.
  • Collaborate effectively with the multidisciplinary team and ensure accurate documentation of patient assessments.
  • Develop and document individualized plans of care, treatment, and services based on patient needs, strengths, limitations, and goals.
  • Facilitate clear communication with patients, families, and the healthcare team while maintaining professional relationships.
  • Evaluate, communicate, and document patient responses to care, ensuring comprehensive patient education and training.
  • Revise patient care plans based on ongoing evaluations and update documentation as required.
  • Assist with patient movement and throughput, demonstrating flexibility to meet organizational staffing needs.
  • Uphold patients' rights by maintaining strict confidentiality and acting as a dedicated patient advocate.

Required Qualifications

  • Associate's degree in nursing or a diploma from an accredited Nursing School.
  • 3-5 years of acute care experience.
  • Current New York State Nursing license.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).

Education & Certifications

  • BS: Nursing (Required)
  • BLS - Basic Life Support - American Heart Association (AHA)
  • RN - Registered Nurse - New York State Education Department (NYSED)

Physical Requirements

This role involves medium work, requiring the exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Frequent walking, standing, or squatting is required.
