FIR Case Manager at Public Health Management Corporation

The FIR Case Manager works directly with participants court stipulated for treatment services with oversight from the Adult Probation and Parole departments. This role ensures that services are provided and documented in accordance with DAP, CBH, Forensic Services, and PHMC requirements. The position provides support to participants while collaborating with all levels of management within the treatment program, Forensic Services, and recovery community.

Responsibilities

  • Establish and manage variable service productivity targets.
  • Provide case management to up to 25 chemically dependent forensic participants.
  • Document service provision within 24 hours of completion.
  • Participate in team meetings, case conferences, trainings, and supervisory sessions.
  • Collaborate with probation/parole, treatment providers, and community agencies.
  • Assist participants in enrolling in various recovery support services.
  • Maintain updated work schedules and records per agency standards.
  • Spend approximately 95% of the work week in the community.
  • Flexible work schedules available based on participant needs.

Experience & Education

Experience in direct case management with the chemically dependent criminal justice population is a plus. Familiarity with fee for service delivery and service note writing is preferred. Minimum education requirement is an Associate’s degree in social services, sociology, psychology, criminal justice, or social work, with 2 years’ experience in public and private human services.
