Housing Case Manager
Project Renewal is seeking a dedicated Housing Case Manager to join their team. This role offers multiple opportunities across various locations in the Bronx, Brooklyn, and Manhattan.
About the Role
The Housing Case Manager will provide comprehensive support to clients, focusing on their housing stability, treatment plans, and overall well-being. This position involves direct client interaction, case management, and coordination with external services.
Key Responsibilities
- Conduct intake, psychosocial, and substance abuse assessments to develop tailored treatment plans.
- Provide case management services to support clients in adhering to individual Service Plans and achieving goals.
- Perform home visits and coordinate medical, psychiatric, and substance abuse services.
- Monitor medication compliance and maintain accurate client records in the AWARDS/CARES system.
- Conduct individual and group supportive counseling sessions.
- Organize and lead educational groups on topics such as money management and employment opportunities.
- Link clients to essential community services.
- Participate in weekly clinical meetings to discuss high-risk clients and track progress.
- Perform other duties as assigned by supervisory staff.
Qualifications
- Bachelor’s Degree in Social Work or a related field.
- 1-2 years of experience working with homeless mentally ill adults or similar populations is preferred.
- Excellent oral, writing, and listening skills.
- Proficiency in computer use.
- Bilingual or multilingual abilities are a plus.
Location & Benefits
- Opportunities available in the Bronx, Brooklyn, and Manhattan.
- Salary Range: $38,000 - $46,000 per year.
