What To Expect (Job Responsibilities)
- Prepare members for telehealth visits, ensuring they feel comfortable and at ease with video onboarding
- Coordinate care by working with home health partners and managing durable medical equipment (DME) and skilled nursing services
- Notify members of lab results and assist in coordinating follow-up care as directed by clinicians
- Advocate for members by addressing care coordination issues through effective communication
- Review internal program referrals and send welcome kits and educational materials to members
What Is Required (Qualifications)
- MA Certification (active or expired accepted)
- Fully bilingual in English and Spanish
- Minimum of two (2) years of patient-facing experience, preferably with telehealth experience
- Prior primary care experience is a plus
- Strong organizational skills to manage a large member panel effectively
