What To Expect (Job Responsibilities)

  • Coordinate care by working with home health partners and managing durable medical equipment (DME) and skilled nursing services
  • Notify members of lab results and assist in coordinating follow-up care as directed by clinicians
  • Advocate for members by addressing care coordination issues through effective communication
  • Review internal program referrals and send welcome kits and educational materials to members

What Is Required (Qualifications)

  • MA Certification (active or expired accepted)
  • Fully bilingual in English and Spanish
  • Minimum of two (2) years of patient-facing experience, preferably with telehealth experience
  • Prior primary care experience is a plus
  • Strong organizational skills to manage a large member panel effectively
