About the Role

Own patient outcomes, not just tasks. We’re hiring Care Advisors to own the patient journey end-to-end. This is not a traditional support role. As a Care Advisor, you will act as a dedicated partner to each patient — guiding them through the healthcare system while actively driving progress in their health. You will combine care navigation, program execution, and behavior change to ensure patients follow through on what matters and achieve meaningful outcomes.

What You Will Do

  • Own the patient journey: Serve as the primary point of contact for patients throughout their program. Build strong, trust-based relationships and understand each patient’s goals, needs, and barriers.
  • Navigate and coordinate care: Help patients find providers, schedule appointments, and access services. Coordinate across providers, referrals, and follow-ups. Remove friction from the healthcare experience.
  • Execute structured programs: Run 8+ week care programs with defined touchpoints and milestones. Ensure patients stay on track with care plans, medications, and next steps. Identify and close gaps in care.
  • Drive behavior change and outcomes: Support patients in building sustainable habits. Hold patients accountable to follow-through. Track progress and escalate when needed.
  • Advocate for patients: Help patients understand their care and options. Act as a consistent, reliable partner in a fragmented system.

Who You Are

  • 2+ years of experience in healthcare, care navigation, customer support, or similar roles.
  • Strong communicator with the ability to build trust quickly.
  • Highly organized and detail-oriented.
  • Able to manage multiple patients and workflows simultaneously.
  • Proactive, resourceful, and execution-oriented.
  • Motivated by improving outcomes, not just completing tasks.

Nice to Have

  • Experience working with Medicare populations.
  • Background as a CHW, MA, health coach, or similar.
  • Experience in care management, virtual care, or digital health.

  • Patients consistently complete scheduled touchpoints.
  • High adherence to care plans and next steps.
  • Strong patient satisfaction and engagement.
  • Clear, accurate documentation to support care delivery and billing.
