Case Manager II - Targeted Case Management
People Incorporated Mental Health Services is seeking a Case Manager II to join our Targeted Case Management Metro Counties team. In this role, you will work with clients diagnosed with chronic mental illness. You will support clients in connecting with resources to live independently and help them develop self-awareness to create positive future plans through regular community meetings.
Main Job Duties:
- Onsite presence within the Twin Cities, Minnesota.
- Conduct functional assessments and develop individual service plans.
- Review and adjust client plans for maximum support.
- Assess client needs and ensure continuity of care with ongoing referrals.
- Maintain detailed records and document all interventions.
- Attend monthly meetings to monitor client services.
Requirements:
- Valid driver's license, excellent driving record, and current auto insurance.
- Bachelor's Degree in behavioral science or related field with 2,000 hours supervised experience OR a Master’s Degree in behavioral science or related field.
- Preferred: Experience with direct services, case management, recovery approaches, motivational interviewing, and person-centered care.

This position is full time, Monday through Friday during business hours, and is based in the Twin Cities Metro Counties with a hybrid work arrangement.
