Job Summary: Pharmacist at PeaceHealth
This Pharmacist position at PeaceHealth is responsible for performing all necessary functions to ensure optimal use of medications to achieve specific outcomes that improve a patient's quality of life. This compassionate pharmaceutical care includes both cognitive and medication distribution related tasks such as dispensing and distributing drugs, advising physicians and patients on proper usage, collaborating with other healthcare professionals to plan, monitor, review, and evaluate patient effectiveness, and monitoring patients for side effects and drug interactions.
Essential Functions
- Reviews medication orders for appropriateness and accuracy, including contraindications, allergies, dose, route, frequency, duplicate therapy, and formulary status.
- Interprets, clarifies, and rectifies physician medication and IV orders, including the medication reconciliation process.
- Interprets, clarifies, and rectifies prescriptions and selects medications for outpatient dispensing.
- Monitors all patients on specified target drugs and initiates pharmacokinetic dosing protocols.
- Evaluates drug therapy for appropriateness and makes drug therapy recommendations to clinicians.
- Prescribes drug therapy per collaborative drug therapy management agreements/Medical Executive Committee approved protocols.
- Dispenses the final product for administration and performs patient counseling.
- Detects, monitors, and reports adverse drug events and medication errors (EIR).
- Supervises and trains pharmacy technicians to assure accurate order entry, preparation, and proper technique.
- Supervises and trains pharmacy interns and externs, and may serve as preceptor as defined by the State Board of Pharmacy.
- Participates in the development of drug protocols and clinical pathways.
- Performs drug usage evaluations on selected drugs and reports results to the Pharmacy & Therapeutics Committee.
- Completes quality assurance auditing as required for prescriptive authority.
- Performs other duties as assigned.
Qualifications
Education
- Bachelor's Degree or Doctor of Pharmacy from an accredited school required.
Experience
- Demonstrated basic pharmacy clinical skills required.
- Demonstrated customer service skills required.
Credentials
- Required: Pharmacist - Washington currently licensed or able to obtain license.
- Preferred: Basic Life Support.
- Preferred: Advanced Cardiac Life Support.
- Preferred: Pediatric Advanced Life Support.
Skills
- Basic computer and keyboarding skills including MS Office applications (Required).
- Ability to identify medications and to read graduation marks on syringes and flasks (Required).
- Strong planning, prioritizing, and organizational skills (Required).
- Excellent communication skills both verbal and written (Required).
- Ability to think and act independently (Required).
- Strong mathematical and analytical skills (Required).
Department / Location Specific Notes
- Northwest Network – St. Joseph Medical Center: Pharmacy Residency Preceptor preferred.
- Northwest Network - PIMC, Whatcom and United: Current Registered Pharmacist license required in the state of WA on date of hire.
Working Conditions
The role involves occasional sitting, squatting/kneeling/bending, frequent reaching, and fine motor skills for precise procedures. Occasional lifting/carrying up to 30 lbs. and pushing/pulling up to 14 lbs. force may be required. Environmental conditions include exposure to biohazard, body fluids, airborne particles, and a noisy environment. Normal vision and hearing (with aids permitted) are essential for communication and accurate information exchange.
