They serve high-need Medicaid recipients through community partnerships with shelters, food pantries, and other organizations, and use a proprietary data-driven technology platform, Arc, for care coordination.

About the Opportunity

This full-time Bilingual Case Manager role is critical in a whole-person, interdisciplinary care model. The role involves direct outreach and engagement with individuals facing serious mental illness, substance use disorder, homelessness, or high medical needs while operating primarily remotely with occasional in-person visits.

What You'll Do

  • Maintain a caseload using evidence-based strategies to promote health goals.
  • Use relationship-based methods to support members with social support navigation.
  • Conduct telephonic and SMS outreach for timely follow-ups.
  • Collaborate with clinical and community teams on care management and barrier removal.
  • Coordinate physical care management appointments and community support referrals.

What You'll Need

  • Minimum 1+ years of work experience; case management preferred.
  • Bilingual in English/Spanish with strong cultural fluency.
  • Comfortable working remotely in a HIPAA compliant space.
  • Strong technical skills with CRM, Excel, Word, email, and video conferencing.
  • Valid driver's license and auto liability insurance.

Preferred Qualifications

  • Experience working with individuals having complex chronic needs, homelessness or mental health challenges.
  • 2+ years of case management experience and familiarity with motivational interviewing.
  • Detail-oriented, organized, and able to multitask in a dynamic environment.
