Overview
Optum Home & Community Care, part of the UnitedHealth Group family, is building an integrated care model that addresses an individual's physical, mental, and social needs. As a team member of the Senior Community Care (SCC) program, you will provide care in nursing homes, assisted living, and other senior housing settings.
Primary Responsibilities
- Conduct comprehensive assessments
- Manage members through acute medical situations or chronic condition exacerbations
- Oversee short term sub-acute rehabilitation episodes
- Ensure accurate documentation including ICD 10 conditions
- Collaborate and communicate with the interdisciplinary care team
- Conduct advanced illness and care planning conversations
- Educate patients and caregivers
Qualifications
Required: Active and unrestricted license, certification as a Nurse Practitioner or Physician Assistant, active DEA licensure or ability to obtain, ability to secure a collaborative practice agreement, physical ability to meet job requirements, driver's license and reliable transportation.
Preferred: 1+ years of clinical experience (long-term care preferred), experience with complex behavioral, social and functional needs, knowledge of Geriatrics, advanced illness discussions, and proficiency in computer documentation.
Compensation & Benefits
This role offers $40,000 Student Loan Repayment or $30,000 Sign-on Bonus (for those new to the program) with a base compensation range from $140,500 to $156,000. Additional benefits include a comprehensive benefits package, incentive programs, equity stock purchase, and 401k contributions.
Additional Information
No on-call, weekends, or holidays required. Employees must remain in role for a set period to maintain benefits. UnitedHealth Group is an Equal Employment Opportunity employer and drug-free workplace.
