About the Role: Clinical Pharmacist - Patient Care

The Clinical Pharmacist - Patient Care is responsible for ensuring the safe and effective use of medications for Optum at Home patients. This includes providing medication therapy management, medication reconciliation, assessing for optimal drug product selection, designing dosage regimens, ensuring effective administration, and offering drug therapy monitoring services.

Activities involve collaborating with providers and other multi-disciplinary team members, applying medication management strategies in alignment with evidence-based medicine. This role offers the flexibility to work remotely from anywhere within the U.S., allowing you to tackle significant challenges in healthcare.

Primary Responsibilities

  • Participate in disease state management programs targeting chronic conditions such as diabetes, heart failure, COPD, and chronic kidney disease. Manage chronic conditions through collaborative practice agreements (dependent on state-specific regulations).
  • Appropriately manage, assess, and/or recommend evidence-based medication therapy based upon the patient's clinical needs for optimizing care outcomes and reducing potential patient safety problems (i.e., drug interactions, suboptimal medication regimens, high-risk medications, etc.).
  • Communicate with providers to discuss pharmacy-related recommendations and benefit options.
  • Design, implement, assess, monitor, and document therapeutic plans utilizing the most effective, safe, and economical medication treatments.
  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
  • Provide education to patients and/or caregivers regarding medications and chronic disease states.
  • Assess open STAR measure care gaps and provide evidence-based recommendations to prescribers, care teams, and patients to close quality gaps.

Required Qualifications

  • Active, unrestricted Pharmacist license in your state of residence.
  • 2+ years of direct patient/member interaction.
  • 2+ years of experience in managed care, ambulatory, medication consultative services, and/or hospital settings.
  • Solid clinical foundation and experience in providing clinical pharmacy services.

Preferred Qualifications

  • Completion of a pharmacy residency in ambulatory care or primary care.
  • Active, unrestricted Pharmacist license in multiple states.
  • Board Certification (BCPS or BCACP).
  • 1+ years of experience with managed care pharmacy benefits.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $44.09 to $78.70 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
