The Clinical Administrative Coordinator is responsible for administrative intake of members or managing the admission / discharge information post-notification, working with providers and the Clinical Team. Includes managing incoming calls, managing requests for services from providers / members, providing information on available network services and transferring members as appropriate to clinical staff. Member outreach to introduce and enroll into the Model of Care Regulatory program, includes coordination of care for these members and work alongside RN / LVN Case Manager. This position is full time (40 hours / week), Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00 am - 5:00 pm PST. We offer 2 weeks of on-the-job training. The hours of training will be aligned with your schedule.Primary Responsibilities
- Respond to patient questions over the phone
- Make patient post discharge calls to identify / close gaps in care
- Make patient calls to engage in Case Management Programs
- Assist with Coordination of Care for Members actively engaged in CM Programs
- Assist with Provider referrals for Case Management
- Connect patients with outside resources (e.g., community resources, social services) to promote self-care
- Other duties as assigned

Required Qualifications
- High School Diploma / GED
- Must be 18 years of age OR older
- Intermediate (2+ years) level of proficiency with Microsoft Office
- 1+ years of experience working in a healthcare office setting
- Ability to type 40+ WPM
- Availability to work any of our full time (40 hours / week), 8-hour shift schedules during our normal business hours of 8:00 am - 5:00 pm PST from Monday - Friday

- 1+ years of experience with working in a customer service role
