About the Division of Family and Child Health
The Division of Family and Child Health (DFCH) of the New York City Department of Health and Mental Hygiene is dedicated to creating and overseeing programs, policies, services, and environments that support the physical and socio-emotional health of New York City families and children. - Apply the nursing process to assess the six prescribed NFP domains, including physical, emotional, social, and environmental needs of women and their families as they relate to health and life course development.
- Assist women and their families in establishing realistic goals and measurable outcomes using reflective practice, stages of change, and other behavioral change approaches.
- Provide counseling to assist women and their families in attaining targeted goals in areas such as prenatal & postpartum care, nutrition, parenting, childcare, family planning, special health problems, housing, continuing education, and employment.
- Evaluate women's and families' progress toward target goals and revise the plan of care as appropriate.
- Develop a trusting and problem-solving relationship with women and their families.
- Assist clients in developing supportive relationships with family members and friends during pregnancy, birth, and childcare.
- Connect women and families with relevant community resources tailored to their specific needs.
- Consult and collaborate with other professionals providing services to women and their families.
- Record nursing activities in a timely manner, adhering to project visit and institutional guidelines.
- Participate in ongoing learning for program implementation.
- Meet with supervisor for a minimum of one hour per week.
- Participate in weekly team meetings and case conferences (2 hours).
- Perform other related duties as needed.
Preferred Skills and Qualifications
- Organized and proficient in multi-tasking.
- A license and current registration to practice as a Registered Professional Nurse in New York State.
- Bachelor of Science degree in Nursing from a regionally-accredited college or university, or one recognized by the New York State Education Department.
- A minimum of two years of full-time work experience in maternal/child health, pediatric health, or a closely related field in a public health, community-based, hospital, clinic, or home visiting setting.
- Must maintain Registered Professional Nurse license and registration throughout employment.
- A Motor Vehicle Driver license valid in the State of New York may be required for certain assignments and must be maintained for the duration of the assignment if required.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook).
