About The Role
The Clinical Care Coordinator is a compassionate, professional individual responsible for supporting patients throughout their care journey. This role requires the ability to manage multiple responsibilities while delivering patient-centered care. The Clinical Care Coordinator educates patients on the SleepCharge program, assists with completion of the sleep checkup, and schedules patients for telehealth visits. Following provider teleconsultations, the coordinator implements physician orders and supports patients in coordinating next steps, including scheduling home sleep apnea testing, initiating CBT-I, and other prescribed services.
This is a remote position. Work schedule will be Monday through Friday 11:30 AM - 8:00 PM EST.
What You'll Do
- Provide patients with sleep charge program information
- Conduct medical intakes
- Contact patients to schedule for telehealth visits
- Conduct telehealth consultations
- Contact patients to schedule home sleep apnea test
- Respond and assist customers with inquiries and/or problem resolution
- Work effectively as a team member
- Comfortable communicating via phone, video conferencing, email and text
Qualifications
- 2 years of customer service experience. (Preferably healthcare field)
- Associates in Medical Assisting
- Great customer service skills
- Ability to problem solve and resolve conflict
- Ability to multi-task
