About the role
Metabolic Health Nurse Practitioner (NP)
Nourish is expanding its clinical model to include clinician-led services as part of our fully integrated metabolic care offering. We are now accepting applications for Metabolic Health Nurse Practitioners for both full-time and part-time roles.
In this role, you will help power a deeply coordinated care team - partnering with registered dietitians (RDs), MDs, DOs, behavioral health therapists, and care operations - to deliver comprehensive, longitudinal support to patients. You’ll provide medical evaluation, diagnosis, and treatment; help guide clinical decision-making across nutrition, lifestyle, and medication pathways; and ensure that patients receive the right intervention at the right time. We handle all administrative, insurance, billing, and scheduling so you can focus entirely on practicing high-quality, thoughtful medicine and supporting meaningful lifestyle and behavior change.

Key Responsibilities
- Provide direct patient care for Nourish patients, many of whom have metabolic conditions, including diabetes, obesity, dyslipidemia, hypertension, and fatty liver disease
- Conduct telehealth visits focused on evaluation, treatment planning, and ongoing management
- Collaborate with RDs and physicians to design comprehensive care plans integrating medical therapy and lifestyle change
- Manage medications and adjust treatment protocols based on clinical data and patient response
- Order and interpret labs, track patient outcomes, and document encounters
- Participate in ongoing quality improvement, care coordination, and clinical innovation efforts
- Serve patients with high standards of personal and professional integrity and ethics, with the ability to practice a high level of confidentiality
Qualifications & Requirements
- Active and unrestricted Nurse Practitioner license (multi-state licensure is strong plus)
- Board certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), or equivalent
- Demonstrated experience in metabolic, obesity, or diabetes medicine is strongly preferred
- Minimum of 2 years of experience managing patients with metabolic conditions (e.g., diabetes, obesity, dyslipidemia, hypertension)
- Strong foundation in evidence-based lifestyle and medical management of chronic disease
- Excellent communication, collaboration, and patient-education skills
- Comfort with telehealth care delivery and modern digital health platforms
- Commitment to compassionate, team-based, patient-centered care

Compensation
Hourly rate: $50-80 / hour
Full-time NPs: Full-time NPs commit to maintaining an average of 30 hours per week (~$75-120K annually at minimum) which makes them eligible for vision, dental, and medical benefits, but can receive additional pay for additional hours (e.g., 40 hours per week equates to ~$100-$160K annually). NPs can scale their hours as desired, with no upper limit on caseload.
Part-time NPs: Part-time NPs commit to maintaining an average of 10-15 hours per week.
