Job Description

Develops and evaluates plan of care

As a Registered Nurse - Float Pool Critical Care Part-time, you will systematically and continuously assess patients' knowledge base and their physical, psychosocial, and spiritual health through comprehensive interviews, physical examinations, and data collection. You will consult with the multidisciplinary healthcare team regarding nursing assessment findings and create individualized written plans of care. These plans will be based on the priority of patient problems, anticipated length of stay, and input from the patient, family, and other healthcare team members. You will communicate pertinent information and changes in patient status to appropriate team members and healthcare providers, collaborating to adjust the plan of care as necessary.

Provides and/or delegates patient care

You will provide and/or delegate patient care according to the established plan of care and protocols, utilizing clinical resources and providing rationale for patient care decisions. Your responsibilities include conducting clinical rounds, providing emergency care, and administering medications. You will operate equipment required for patient care according to manufacturer's recommendations and perform or assist with procedures and treatments such as changing dressings, feeding patients, and providing pre/post-operative care. Adherence to and monitoring of infection control standards, alongside promoting and ensuring patient safety in accordance with state and federal guidelines, is paramount.

Documents patient care

Accurate and timely documentation of patient care on all appropriate forms is essential. This includes entering the date and time of entry, signing your name legibly on the patient's chart, and completing the Nursing Admission Assessment within 24 hours of admission. You will document the patient's response to interventions and the plan of care on the Progress Notes, and perform vital signs and pain scores at least every four hours. Transfer Assessments for patients transferred out of the unit and Perioperative Assessments for patients scheduled for surgery or procedures must be completed. Additionally, you will complete Frequent Observations and the Intake/Outtake Record according to established protocols and initiate/complete healthcare referrals in collaboration with the interdisciplinary team.

Provides patient education

You will identify patients' learning needs upon admission, informing and educating both patients and their families to enable their active participation in care and decision-making. Developing and implementing a teaching plan of care, conducting patient teaching conferences, and thoroughly documenting all patient teaching are key aspects of this role.

Participates in Quality Improvement process

Participation in the Quality Improvement process is expected, including contributing to the development and implementation of unit goals and objectives. You will attend in-service programs on new products and equipment, complete occurrence reports according to policy, and perform unit-based audits as assigned. You will also participate in identifying problems requiring investigation and in developing and implementing corrective action plans for the unit.

Participates in unit staff meetings and clinical conferences

You will attend unit staff meetings and unit-based clinical conferences as scheduled, and may be assigned to represent the clinical unit on committees. Annually, you will submit a record of continuing education and a self-assessment of your clinical performance.

Performs charge nurse and preceptor responsibilities

As assigned, you may perform the responsibilities of a charge nurse and preceptor, adhering to established protocols. You will also perform other related duties as required.

Job Qualification

  • Graduate from an accredited School of Nursing.
  • Bachelor's Degree in Nursing, preferred.
  • Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN degree within five (5) years of employment date.
  • Current license to practice as a Registered Professional Nurse in New York State.
  • BCLS, ACLS or PALS, required.
