Registered Nurse Float Med Surg Tele
Northwell Health is seeking a skilled and compassionate Registered Nurse (RN) to join our Float Pool, specializing in Medical-Surgical and Telemetry units. This role is crucial in providing high-quality patient care across various units, ensuring a consistent and excellent standard of service.
Job Responsibilities
- Develop and implement age-specific care plans using the nursing process (assessment, diagnosis, planning, implementation, evaluation).
- Collaborate with physicians and healthcare teams to coordinate procedures and treatments.
- Utilize leadership skills and clinical judgment to manage patient care and direct unit activities.
- Conduct patient interviews, explain procedures, and address patient/family concerns.
- Assess various health statuses including gastrointestinal, cardiovascular, respiratory, renal, and neurological systems.
- Evaluate mobility, sensory deficits, prostheses use, and skin condition, adjusting care plans accordingly.
- Manage pain assessment and treatment effectively.
- Communicate and document patient's physiological and psychological health status and care plans.
- Identify patient outcomes and develop criteria for measurement.
- Address actual and potential patient problems and needs for teaching.
- Develop patient outcome statements and establish individualized patient goals.
- Coordinate the cost-effective use of supplies, equipment, and medications.
- Ensure patient safety through constant vigilance and adherence to precautions.
- Administer prescribed treatments and medications accurately and safely.
- Use necessary equipment correctly and efficiently.
- Provide emotional support to patients and their significant others.
- Maintain accurate documentation of patient treatments, activities, conditions, and responses to interventions.
- Act rapidly and effectively during emergencies, managing self, patients, and other employees.
- Foster a calm and restful environment.
- Communicate effectively with patient families and caretakers.
- Participate in discharge planning and coordinate referrals.
- Provide discharge instructions and evaluate teaching effectiveness.
- Perform variance analysis for performance improvement and design/implement systems to enhance care.
- Collaborate with care team members and physicians on treatment plans.
- Assist physicians during treatments and diagnostic procedures.
- Keep Patient Care Manager/designee and physicians informed of patient status changes.
- Delegate assignments and ensure timely completion of patient care tasks.
- Perform Decubitus Care (Grades I-IV) and preventive skin care measures.
- Apply dressings, compresses, and perform irrigations according to facility procedures.
- Educate patients and families regarding medications, drug/food interactions, and conditions.
- Recognize and utilize health teaching opportunities and resources.
- Provide for early discharge planning and appropriate referrals for post-hospital care.
- Perform other related duties as required.
Job Qualifications
- Graduate from an accredited School of Nursing.
- Bachelor’s Degree in Nursing (BSN) is preferred.
- Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of the job entry date.
- Current License to practice as a Registered Professional Nurse in New York State required.
- Specialized certifications as needed.
