Key Responsibilities
- Perform psychosocial assessments of patients and families.
- Provide introductions and orientation to newly admitted patients and families.
- Evaluate new admissions through interviews and external agency contact.
- Prepare comprehensive psychosocial assessments and care plans.
- Participate in interdisciplinary team meetings and collaborate with teams.
- Maintain thorough documentation of patient contacts and assessments.
- Formulate and implement psychosocial treatment and discharge plans.
- Provide counseling to individuals and families coping with illness and life changes.
- Utilize knowledge of community resources and entitlement programs.
- Negotiate community systems and programs to benefit patients.
- Implement and evaluate treatment plans continuously.
- Ensure social work services meet required standards.
- Apply knowledge of financial, reimbursement, and regulatory systems.
- Complete all required forms and documentation accurately.

Qualifications
- Master’s Degree in Social Work, required.
- Licensed Master Social Worker (LMSW), required.
- Minimum of three (3) years of social work experience in an acute care hospital setting, required.
- Specialized experience may be required based on assignment area.
- Current License to practice as a Clinical Social Worker in New York State, required.
