Housing Stability Case Manager - Riverbed (Short-Term)

This is a short-term, grant-funded project expected to conclude on December 31st, 2026.

NAMI San Diego & Imperial Counties (National Alliance on Mental Illness) is a nonprofit organization dedicated to improving the quality of life for individuals and families affected by mental illness through free education, support services, and advocacy.

Primary Responsibilities/Function

The Housing Stability Case Manager for the Riverbed Encampment Resolution Program provides specialized case management to unhoused and recently housed individuals from the Riverbed Encampment community, with a focus on mental health. As part of a team, you will develop person-centered plans, assist clients in securing housing of their choice, and then focus on building skills to help them maintain their housing independently.

Duties and Responsibilities

  • Conduct daily in-person field outreach or visits to recently housed neighbors.
  • Respond rapidly to self-referrals and referrals from providers and the community.
  • Link clients to housing and community supports, providing case management and transportation based on their goals.
  • Refer and coordinate shelter and program placements, engaging in care coordination.
  • Assist clients in obtaining necessary documents for housing and employment.
  • Ensure clients have health insurance, are connected to primary care, and receive mental health and SUD treatment as needed.
  • Work towards ensuring clients have sustainable income.
  • Develop and maintain positive landlord relationships.
  • Support clients in family mediation and reunification coordination.
  • Collaborate with local service providers for a seamless transition into interim and permanent housing.
  • Manage a caseload of up to 20 active program participants.
  • Provide client support for up to 2 years to achieve their goals.
  • Develop reasonable step-down plans for caseloads to ensure long-term housing stability.
  • Document all case management services and client progress in the HMIS Clarity system.
  • Drive company or personal vehicle as required.
  • Perform other duties as assigned.

Accountability

Reports to the Director of Independence & Housing.

Physical Demands

Employees must be physically present at the worksite during scheduled hours. This role involves prolonged periods of standing and walking outdoors daily, potentially in canyon terrain and inclement weather. It also includes prolonged periods of sitting at a desk and computer work. The ability to lift up to 30 pounds may be required.

Minimum Requirements

  • 1-2+ years of experience in case management.
  • Satisfactory driving record and willingness to operate company vehicles.
  • Effective verbal and written communication skills.
  • Proficient computer skills (Microsoft Office, Word, Excel) and web navigation.
  • Knowledge of HMIS or other case management systems preferred.
  • 1-2 years of experience in housing case management preferred.
  • Knowledge of San Diego County resources is a plus.
  • Demonstrated ability to maintain confidentiality.
  • Consistent and reliable attendance.
  • Preferred lived and/or work experience in areas such as substance use disorders, healthcare navigation, justice involvement, Transitional Age Youth (TAY), harm reduction, intellectual and developmental disabilities, family with minors housing, refugee/asylee navigation, LGBTQIA2S+ services, San Diego County housing subsidies, SSI applications, IPS employment, and financial literacy.
  • Background check required after contingent offer of employment, with satisfactory results prior to final offer.
