Job Description: Adult ICU New Graduate Fellowship Clinical Nurse
The Clinical Nurse at Mount Sinai Morningside is a Registered Professional Nurse dedicated to providing safe, competent, and high-quality care. This role is grounded in nursing theory and research, serving a designated group of patients and their significant others.
Patient Care Responsibilities
- Delivers patient care integrating Relationship Centered Care and applying the nursing process based on theory, research, evidence-based practice, and organizational standards.
- Assesses patient care needs and applies critical thinking for effective management.
- Integrates relevant assessment and intervention skills in daily nursing care.
- Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate patient care plans.
- Prioritizes all aspects of patient care, including teaching, rounding, coaching, planning post-hospital care, and delegating appropriately.
- Educates patients and caregivers on their plan of care, transition of care, health promotion, and disease prevention, anticipating their needs and readiness to learn.
- Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team.
- Manages assignments within the Modified Primary Nursing Care Delivery Model and effectively allocates material resources.
Patient Experience Focus
- Role models Mount Sinai Relationship Centered Care tenets through actions like Admission Welcome, Bedside Shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain, Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration.
- Considers patient values, preferences, expressed needs, and knowledge in all aspects of care.
- Establishes and maintains therapeutic relationships with patients and families.
- Uses evidence-based practices to enhance understanding of patient perceptions of care.
- Initiates service recovery for patient and family concerns, escalating to management or Patient Relations as needed.
- Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions.
- Supports staff in using evidence-based practices to improve understanding of patient perceptions.
- Conducts Purposeful Hourly Rounding (PHR) on assigned patients, addressing the 4 Ps.

- Practices safe and efficient patient-centered care, identifying and escalating barriers and improvement opportunities.
- Demonstrates clinical skills and knowledge crucial for quality and safety in the patient population.
- Implements process improvement strategies based on institutional, evidence-based practices and procedures.
- Engages in formal and informal peer and institutional review processes.
- Collaborates with interdisciplinary teams on quality improvement projects, evidence-based practice activities, and nursing research studies.
- Contributes to quality and safety practices and required compliance measures.
- Implements high reliability principles to identify and avoid high-risk behaviors for a safe patient environment.
- Escalates potential safety hazards or gaps from best practice following institutional chain of command.
- Utilizes institutional information technology for documenting nursing practice to support quality and performance improvement initiatives.
Operations & Resource Management
- Performs charge nurse duties as assigned.
- Coordinates with Clinical Coordinator/Nurse Manager to ensure appropriate supply utilization and sufficient inventory.
- Escalates material or staffing resource deficits to charge nurse or management.
- Demonstrates patient-focused and cost-effective approaches to patient care regarding equipment, staff, supplies, and other resources.
- Documents accurately and thoroughly in compliance with hospital, regulatory, and legal requirements.
- Facilitates patient throughput on a shift basis.

Qualifications
Candidates for the Adult ICU New Graduate Fellowship Clinical Nurse position must meet the following criteria:
- Graduation from an accredited Nursing program.
- Bachelor’s Degree in Nursing (BSN).
- Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year.
- Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO.
- Relevant clinical competence in the assigned nursing practice area; new graduates must possess current knowledge of the nursing process and its application.
- Licensed as a registered nurse with current registration in New York State.
- Certifications required: Basic Life Saver (BCLS) from AHA. ACLS certification is required for ICU units.
